I understand ALL future billing and correspondence will be sent to the above address as noted and will remain in effect unless notified in writing of any change. I also understand that I am responsible for ALL water and sewer billing amounts (including late fees & service fees) due associated with above named property. I am aware that water service to the property will be shut off for non-payment of delinquent account and service will not be restored until payment is made in full. I understand I must keep my current address/phone number on file with this property's account. Note, your original application will remain on file, this form is for mailing/correspondence change only.
